ABOUT VACANCY:
We need a strong PHP (Laravel) developer who’s excited about AI and ready
to build the backbone of a modern healthcare platform. You’ll spend most of your
time architecting backend features, building APIs, and optimizing performance —
but you’ll also integrate AI agents and LLMs to make our platform smarter.
MAIN TECHNOLOGIES:
* Backend: PHP 8.x, Laravel 10+
* Database: MySQL/PostgreSQL
* AI: Claude API, OpenAI API, Python for ML tooling
* Tools: Docker, Redis, Queue workers
* Frontend: (whatever you use — Vue/React if applicable)
Responsibilities:
Core Backend Work (70%):
* Develop and maintain PHP (Laravel-based) backend services
* Design RESTful APIs for our telemedicine platform
* Build secure patient data management systems (HIPAA/PIPEDA compliant)
* Optimize database queries and system performance
* Implement real-time features (chat, notifications)
* Work with our appointment scheduling and directory systems
* Write clean, testable, maintainable code
AI Integration Work (30%):
* Integrate LLM APIs (Claude, GPT, Medgemma, etc)
* Build AI agent pipelines for clinical automation
* Implement prompt engineering strategies within our backend
* Create intelligent routing and triage systems
* Develop AI-powered features (symptom analysis, documentation assistance)
* Manage model API costs and performance monitoring
Requirements:
Must Have:
* 4+ years of PHP development, 2+ years with Laravel specifically
* Strong understanding of MVC, OOP, and design patterns
* Experience building RESTful APIs and microservices
* Solid database skills (MySQL/PostgreSQL) and query optimization
* Understanding of authentication, authorization, and security best practices
* Experience with Git, CI/CD, and modern development workflows
* Comfortable working with healthcare data and compliance requirements
Important for This Role:
* Hands-on experience integrating LLM APIs into production applications
* Working knowledge of Python for AI tooling and scripts
* Familiarity with AI coding assistants (Claude Code, Cursor, Copilot)
* Understanding of different LLM models, their capabilities and trade-offs
* Experience with prompt engineering and managing AI outputs
* Basic knowledge of AI agent architectures and workflows
Bonus Points:
* Previous healthcare/telemedicine platform experience
* Experience with real-time systems (WebSockets, Laravel Echo)
* Knowledge of medical data standards (HL7, FHIR)
* RAG implementation experience
* Contributions to open-source Laravel or AI projects
* Experience scaling Laravel applications
